Position:     Housing Case Manager – UW-Medicine

Hours:         Full time, limited evenings/weekend work required

Reporting:   Housing case managers report directly to the Homeless Services Program Director

 

Description:

Housing case managers provide services and financial assistance to program participants within our UW-Medicine Program. UW-Medicine is a program designed to provide housing case management to individuals facing a wide range of housing instability. This program works with patients who receive specific healthcare services from the UW Department of Medicine and who require stable housing in order to successfully manage their healthcare needs. Program participants are referred directly to this program from the UW Department of Medicine.

Job Duties:                           

  • Provide housing case management focused on securing housing stability with program participants who are either experiencing homelessness or housing instability
  • Employ a housing-first strategy and philosophy when working with participants
  • Connect with landlords in the community in order to assist participants in finding their own housing
  • Create individualized housing plans and set goals with participants; have flexibility around participant needs and update plans as appropriate
  • Connect participants with other agencies that can provide non-housing-related services as needed
  • Maintain case notes of participant meetings and record all services provided to participants in the homeless management information system (HMIS)
  • Remain up to date on HMIS trainings
  • Understand and ensure compliance with detailed program regulations regarding services provided and documentation collected
  • Attend regular supervision, case management meetings, and relevant trainings
  • Receive training on other homeless services case management programs within Tenant Resource Center as assigned and maintain competency in those programs; provide services to participants from other programs as needed and fill in for other case managers when they are out of the office
  • Receive housing counseling training and update training as needed (at a minimum of once per year) in order to provide well-informed housing services to program participants

UW-Medicine Program Specific Responsibilities:

  • Provide housing case management (for up to 24 months) focused on either moving program participants from homelessness into housing or stabilizing participants who are at risk of losing their current housing; manage a caseload of up to 30 individuals
  • Work with participants throughout Wisconsin who are primarily located in Dane County or Rock County; travel to Rock County approximately twice a month
  • Meet participants where they are; meetings with participants could occur in their home, our office, outside, in the clinic of their medical provider, at other organizations/offices, or in other spaces as needed
  • Receive referrals directly from the UW clinic and ensure an appropriate level of service to all referrals
  • Provide a housing needs assessment to all referrals to ensure each participant is receiving the correct level of care
  • Design individualized rent subsidies to fit each participant’s financial needs
  • Use your own vehicle or a vehicle provided by Tenant Resource Center in order to transport participants to housing-related appointments
  • Collaborate with medical case managers to provide strong wraparound services

Requirements and Qualifications:

  • BSW, related degree, or 1 year of homeless services/case management experience required; applicants with MSW or related degree preferred
    • Applicants who do not hold educational or experimental requirements but believe they hold necessary skills from other means are encouraged to apply
  • Spanish language speakers strongly encouraged to apply
  • Ability to hold compassion and empathy for all people
  • Desire to provide client-centered care to all program participants
  • Understanding of harm-reduction practices, motivational interviewing skills, and trauma-informed care
  • Ability to manage affect and maintain professional boundaries
  • Ability to remain calm when responding to potentially life-threatening crises
  • Organizational and time management skills
  • Strong communication skills; especially skilled at taking complicated information and making it more accessible
  • Previous experience working within databases and ability to quickly adapt to new computer program reporting tools
